SIBO symptoms in women, and why they are so often missed
SIBO is diagnosed far more frequently in women than in men, and the reasons are closely tied to hormones. Oestrogen and progesterone both influence how quickly the gut moves food through the small intestine, so the risk of bacterial overgrowth shifts across the menstrual cycle, during pregnancy, and again around perimenopause and menopause. That is exactly why female SIBO symptoms are so often written off as hormonal, and why getting the right test matters.

Why SIBO symptoms in women are tied to hormones
The small intestine relies on a repeating wave of muscle contractions, known as the migrating motor complex, to sweep leftover bacteria through to the colon between meals. Progesterone relaxes smooth muscle throughout the body, including in the gut, which can slow this sweeping action down. That is one reason bloating and constipation are so commonly reported in the second half of the menstrual cycle and during pregnancy, when progesterone is at its highest.
Thyroid function adds another layer. Hypothyroidism, which is significantly more common in women, also slows gut transit and is frequently found alongside SIBO. And conditions with a strong hormonal component, such as PCOS and endometriosis, are associated with altered gut microbiomes and a higher reported overlap with SIBO-type symptoms. That overlap is part of why bloating in these conditions is so often dismissed rather than properly investigated.

- Bloating that follows a monthly pattern, or a distended “SIBO belly” that worsens through the day
- Constipation or loose stools that track with your cycle phase
- Abdominal pain, excessive wind and audible gurgling after meals
- Digestive symptoms that began or worsened during pregnancy

- Fatigue, brain fog, low mood or anxiety alongside digestive symptoms
- Nutrient deficiencies such as iron and B12 that do not fully resolve with supplementation
Hydrogen and methane SIBO symptoms
Not all SIBO presents the same way, and the gas involved makes a practical difference to treatment. Hydrogen-dominant SIBO tends to produce looser stools, urgency and diarrhoea. Methane-dominant overgrowth is more strongly associated with constipation, a harder distended abdomen and slower transit, and it is the pattern we more often see in women whose bloating never quite settles. Hydrogen sulphide SIBO is a third pattern, typically involving sulphurous wind and diarrhoea. A three-hour breath test tells us which one you are dealing with, rather than leaving you to guess.

Is SIBO more common in women than men?
Yes. SIBO is diagnosed more frequently in women, and the leading explanation is hormonal. Progesterone slows the muscular contractions that clear bacteria from the small intestine, so risk rises at points in life when progesterone is high or when oestrogen is falling.
Can SIBO cause hormonal symptoms?
The relationship runs in both directions. Bacterial overgrowth affects how oestrogen is metabolised and recirculated, and it can drive the fatigue, brain fog and low mood that are often attributed to hormones alone. Testing both is the only way to separate the two.
Can perimenopause cause SIBO?
Perimenopause does not cause SIBO directly, but falling oestrogen is linked to lower stomach acid and slower gut motility, and both make bacterial overgrowth more likely. New bloating or reflux in your forties or fifties is worth testing rather than assuming it is simply hormonal.
