Acne is often treated as a purely topical problem, but persistent breakouts are usually a sign that something deeper is out of balance — whether that’s hormonal fluctuation, gut health, inflammation or a combination of the three. Rather than focusing only on the skin’s surface, we look at what’s driving the breakouts so treatment addresses the cause, not just the symptom.

What causes acne?
Acne develops when hair follicles become blocked with excess oil (sebum) and dead skin cells, creating an environment where bacteria can thrive and trigger inflammation. What causes that blockage and inflammation in the first place, however, is rarely just a skincare issue. Hormonal fluctuations are one of the most common drivers — shifts in androgens can increase oil production, which is why acne so often tracks alongside puberty, the menstrual cycle, pregnancy or conditions such as PCOS.
Gut health plays a significant role too. An imbalanced gut microbiome can drive systemic inflammation and affect how the body regulates hormones and processes toxins, both of which can show up on the skin. Blood sugar regulation is another frequently overlooked factor — diets high in refined sugar and processed carbohydrates can spike insulin, which in turn stimulates the same androgen pathways linked to oil production. Add to this chronic stress, food sensitivities, nutrient deficiencies and environmental toxin exposure, and it becomes clear why acne rarely responds to a one-size-fits-all approach.

Treatment approach
Why symptom management alone isn’t enough
Conventional acne treatment tends to focus on the skin itself — topical retinoids, benzoyl peroxide and antibiotics to reduce bacteria and inflammation, or in more persistent cases, hormonal treatments such as the contraceptive pill or isotretinoin. These can be genuinely effective at clearing breakouts while they’re being used, but they’re treating the visible result rather than what’s causing it.
That’s why acne so often returns once a course of antibiotics or topical treatment ends — the underlying imbalance was never addressed.
